Jake Forker enters his second year with the Cougar Baseball program as the athletic trainer in 2007. In that role, he supervises the daily treatment of student-athletes, oversees the rehabilitation progress of injured players and maintains the team's medical records.

He also works closely with the Cougar Football program during the fall while studying towards his master's degree in sports administration at UH.

Forker joined the UH program in January 2005 after earning his bachelor's degree in kinesiology from Texas A&M in December 2004. As a student in the A&M Sports Medicine department, he worked with the men's basketball, football, track and field, swimming and men's tennis teams.

The Sugar Land, Texas, native also has worked with the NFL's Houston Texans in different capacities. Forker started with the team as a scouting department intern in 2000 before adding duties with football operations the next year. In 2002-03, he assisted the Texans' sports medicine staff as a student athletic training intern.

Forker is a nationally certified member of the National Athletic Trainers' Association and state-licensed athletic trainer. He also is a member of the National Athletic Trainers Association, Southwest Athletic Trainers Association and the American College of Sports Medicine.

He earned Verizon Scholar Student-Athlete awards in 2000 and 2004-05 and served as a student-athletic trainer mentor at Texas A&M in 2003.

Born on July 16, 1981, Forker lives in Sugar Land. He recently became engaged to former Texas A&M All-America swimmer Ewelina Zwierzykowski with plans to marry in the summer of 2008.
